MNREM Sub-Grantee Information and Reports

Below is the Sub-Grantee Guide, and the reporting forms that you will need for your quarterly reports. All reports are due 15 days after the end of the quarter.

Sub-Grantee Guide - (August 2009 Modification) Includes a Grant Agreement Overview, process for requesting payment, reporting schedule, and report samples.

Quarterly Narrative Report - MS Word document to be used for submitting your quarterly narrative report

Monthly Fiscal Report/Request for Payment - Excel document to be used for submitting your monthly fiscal report, as well as to request payment (see instructions in the Sub-Grantee Guide)

Monthly Leveraged Resources Report - MS Word document, to be submitted as match/ leveraged funds backup with every fiscal report and/or request for payment

Monthly Expenditure Projection Report - Excel document, to be submitted each month along with your financial report and leveraged funds report, projecting your monthly expenditures through the end of your MNREM Grant Agreement. 

Budget Modification Request Guide - MS Word document, to be used as a guide if you are requesting a modification to your grant budget

Contract Extension Request Guide - MS Word document, to be used as a guide if you are requesting a no-cost extension to your MNREM grant-funded project (only to be requested, and will only consider, under most extreme circumstances)

Addition to Final Narrative Report - Instead of a final report, please put some thought into these areas and address them in your final narrative report.
